DEADLINE FOR REGISTRATION: SEPTEMBER 3, 2010. * All fields are required to process your reservation registration. Please fill out all fields. When you are finished press the Submit Button at the bottom of the page. An information page summarizing what you have submitted will automatically be sent to the Smoke on the Water organizers and to the email address you furnished. Please print out this information page for your records. Registration fee must be paid by credit card. Checks not accepted. SET UP TIME IS 9:00 a.m. to 2:00 p.m. ON FRIDAY, September 17.
NO EXCEPTIONS WILL BE MADE. IF NOT REGISTERED BY THE 2:00 p.m. DEADLINE, September 17, 2010, YOU'RE SPACE WILL BE ELIMINATED WITH NO REFUND. WAIVER OF LIABILITY:In consideration of your “submission or acceptance” of this entry, I, the undersigned, Intending to be legally bound, hereby from myself, my heirs, executors, and administrators waive and release any and all rights and claims for damages against the Smoke on the Water BBQ Cook-Off Committee, State of Arkansas, City of Pine Bluff, their agents, successors and assignees for any and all injuries suffered by me in this event. Further, I hereby grant full permission to the Smoke on the Water BBQ Cook-Off Committee and/or any and all agents authorized by them to use any photographs, video tapes, motion pictures, recording and any other record of this event for any legitimate purpose.

INSURANCE: Please fax or send hard copy of your insurance to: Smoke on the Water Productions, Inc. 621 East 8th Avenue Pine Bluff, AR 71601 (870) 536-8910 REGISTRATION CONFIRMATION BY E-MAIL Confirmation of team application will be forwarded to you by email to the address given in form. Please make sure address is correct and valid. VENDOR PASSES TO BE MAILED 2 WEEKS BEFORE FESTIVAL Each vendor will receive 2 vehicle passes only. Pass must be displayed for entry to park. After setting up, all vehicles will be required to park in designated area. No exceptions. NOTE: NO REFUNDS ON REGISTRATION. In a case of cancelation, the registration amount will be applied to the next available contest that the team can participate in.
